Expense Report Approval Workflow - Accounting Approval and Supervisor Approval fields not showing.

I've been working on an approval workflow for Expense Reports and somewhere a long the way the two checkboxes for supervisor approval and accounting approval have disappeared.
Both are currently set on the form to normal and I've tried to set their display type in the workflow but the workflow is having some odd issues. For some reason it's able to target the fields to update their values, but not their display type.
I'm really not sure why this is happening.
Also, I would expect that once the report has been approved by both supervisor and accounting, that sets the report to fully approved, but it still says 'awaiting approval' even though whilst not visible, both boxes should be checked at this point. I suppose I could just set the field 'Approval Status' to approved, but does this typically happen once supervisor has approved or both supervisor and accounting?
