Why does my bank upload keep failing?

We were told that the automatic loading of the bank data breaks all the time and to manually upload it. When netsuite does it, it works fine. But, when I do it, it always fails. I have tried all types of files and don't know what else to do. I saved my file as CSV UTF-8. Thank you for any help. Gail
