Why Duplicate Email Are Sent when "Send on Update" is Checked for Saved Search Email Alert

We have a saved search that is set to "Send on Update" as below.
This particular saved search is for SO approval under certain criteria. When the SO is approved by clicking the "Approve" button, each of the receipts receive 2 identical alerts.
However, when the SO is approved by selecting the changing Status field (below), each recipient receive only 1 alert.
Why duplicate emails were sent under the first scenario but not the second? How can I modify the saved search so that only one alert is sent under the first scenario?
