Security Question - after every login

Hi everyone!

We have a few employees that are prompted each time logging into NetSuite to enter their security questions. They are not changing locations/browsers, clearing cache/cookies, or using incognito browser. We are not sure why they are prompted each time and it seems to happen to a certain select employees randomly. One employee it just started happening too.

Any thoughts on how to fix this or what settings maybe prompting this issue?
