Performance Document missing

I would like to create a performance document for certain employee where only the questionnaire appears. The employees would complete the performance document by answering the questions. So far I have not been able to successfully create this type of performance document.
In my attempt, I have created and submitted a performance document containing only the 'Manage Participant Feedback' task. This performance document was received by the correct employees, however, when an employee clicks on the task, the 'Feedback Requests' page opens but the questionnaire does not appear.
I will attach my settings. Any suggestions or ideas would be sincerely welcome.
