Capturing EEO (Race/Gender/Ethnicity) post offer

Hi,
I wanted to try to see what other companies are doing to capture EEO information at a post offer step/status or for the hired candidate. We have all US candidates complete EEO information during their initial application, but we have requirement to gather this information again for the selected candidate.
Trying to understand the best way to do this. In my initial research, I see that one option might be to create a 2nd application, have recruiters post that req to that 2nd career section that would be set to private, and then send correspondence to candidates that are selected with a link that would take them directly to their POST offer EEO information application flow.
