Data Extract - Job Offer

I am looking for a sample of a data extract for recruiting specifically job offers. We are implementing a Occupational Health software. A candidate does not have a person number but a candidate number. If a candidate has been extended a job offer and they have accepted, I need the candidate number, first & last name, DOB and NID(SSN). This limited information was be interfaced to the Occupation Health for medical to do drug testing. When recruiting does the Move to HR that candidate gets a person number.
