Exception count is not considering all seeded exceptions

Content (required):

We have defined a table with 2 measure, Exceptions: Quantity and Exceptions: Count. Hierarchy includes only organization and exception type. We have not selected any members. After running the plan, even though there are several exceptions, none of them are showing up in this table.

We then created 2 custom measure based exceptions and they show up in this table but the seeded exceptions are not showing up.
