How to fix missing nav bar after proxy login?

After performing a proxy login, the nav bar that contains the proxy logout link is missing entirely.

Since the proxy logout icon and all the other nav-bar icons are missing, I must clear cache and cookies in order to get back to my own account. I am a full access user and a relatively new member of the CPQ team. I have been unsuccessful, so far, in determining what is causing the nav-bar to disappear. I suspect something may have been added/changed by a past member of this team, but am unsure on where to start looking.
