Action checks are not working in OTM

Summary: I have added new action checks for a user role and it is not working. I was able to perform all the actions regardless of action checks. I made sure the object I was working on was not part of the allowed condition I had created. I realized later that it is not a new action check, the issue is with all the existing action checks in OTM for that user role.

Please advise what needs to be done, and what I need to check. This is happening only in the test instance. DEV and PROD are working as expected.
