No integration instance created for Oracle Field Service Activity

I am working on integrating Oracle Customer Care and Billing (CCB) with Oracle Field Service (OFS) using Oracle Integration Cloud (OIC). The integration works as expected. However, I do not see integration instances created in OIC for OFS activities in rare cases. This is resolved by restarting the integrations in OIC. In few cases, some of the OFS activities were entirely missing and unable to recover.
Any idea what causes it? Also, what is the way to recover any OFS events not available in OIC?
