Help with Positive Pay File format and data definitions

Summary:

We have a positive pay file generated and it works however the fixed width format is a bit off and the bank is not processing it because it does not match the requirements. I need to hack an existing file but am unfamiliar with the how the existing file works. It does not make sense in terms of what is called position and length. I have output and it does not match what I think the position and length interpretation. I have attached the positive pay file format and the redacted output of our positive pay
