import costing mapping sets

Summary:

I am trying to impost new mapping into the COA. I have export the template and filled in the same data I use to manually key this information. I also export the mapping we have today so I filled in these exact same columns. I am getting an error in import telling me "The output value in the output column is required. What is the system looking for here?
