Create Information Template Attribute List

Summary:

I'm trying to create a new information template, we already have one created that asks the users to specify which business they work for. I need another one so I can apply it to a different set of categories.

Below is an image of the area I'm on about. Basically when I click that attribute list, I want to have another option under employee/business unit.
