Users Are Receiving Requests to Receive PO's After the PO has Already been Received and Closed?

We're noticing that users are receiving receipt confirmation notifications when a PO has already been received. In the example below, the receipt was entered by the requestor's manager on 6/15, but the notification was escalated 6/19, after the receipt was entered. Why is this happening?
