Tasks on Legacy Onboarding sub-process appear to loop

In a very small number of cases tasks within one of our process fire twiceContent
We have a subprocess containing tasks that are scheduled to fire on a candidate's start date so the condition 'is 'greater than or equal to start date'.
On a very small number of cases ( I know of 4 in the last 2-3 months) these tasks appear to be created and then within about 10-15 seconds a second batch of the same tasks are created. The first batch of tasks appear to be created and complete almost immediately even though the assignee has not viewed them yet. The second batch that are generated are the 'real' tasks as they wait for the assignee to actually complete them and therefore remain In Progress. Duplicate tasks are also showing in task lists. Wondered if anyone had come across this
