There have been more than a few questions around this topic in the recent past. My colleague Sumana has compiled some tips that help elucidate the concept of how Top Worker configuration is to used. I am providing the details from a post in Purchasing where Sumana offered these details in response to a customer question.

The top worker is required to compute the hierarchy up to which level routing should happen (used for supervisory and job level routing). Task won't be routed up beyond this user.
Note that the number of approver level constraint should not cross the number of users in the hierarchy, otherwise it will result in error.
