Instructor feedback dashboard

I am trying to build a dashboard similar to the one that is in Oracle demo instance. It is called Instructor Feedback Dashboard but I am not getting the source of the data.

Hello,
For a client, we want to build a dashboard that is similar to what we have in Oracle demo instance. We just need to make few changes to that dashboard. When I am trying to see the responses to questions in HRQ_QSTN_RESPONSES, I am not able to find the comments that we have on the dashboard in that table. Neither when I check from the learner's account, I am able to see that he has given feedback on any course but this appear on the dashboard. I want to understand, how this dashboard has been built and what is the
