Learning how is it possible to have survey for a manager /HR / class instructor?

Hello ,

In learning is it possible to have information regarding the manager evaluation /survey of the learning system ? Or/and the class instructor? 

we have courses with evaluation in the course level, but we would like a manager or/and instructor of a class to fill survey for the learning in general (or if not possible only for courses/classes )
