Redwood 'additional info' section presenting error in manager evaluation of worker

I am trying to submit a manager evaluation of a worker but the additional info section in the document keeps coming up with an error and will not allow me to submit, help!

I set up a performance review for employees and managers to rate their proficiencies against their job profile competencies. I had the manager bypass the employee self evaluation and had the manager complete their evaluation task. Once complete, I went to submit and the page sort of froze/processed slightly and nothing changed on the page. I scrolled down to the bottom of the document and saw there was an error in the additional info section but it offers no context. Has anyone experienced this before? Is this a redwood issue?
