New User Immediately Set to Inactive

When we create a new User associated with an employee, occasionally it is immediately set to Inactive once we save the record.

Hello Experts,
We are seeing an intermittent issue where newly created user accounts are set to "Inactive" once they are saved. It doesn't happen all the time, but we are seeing it with a bit more frequency and I'm not sure how to troubleshoot this. There is seemingly no difference on the front end when comparing the associated employee records and the employees are active and User record has a role.
