Can we limit what an employee is asked to feedback on, when using participant feedback?

Summary:

I've just watched a video on Oracle Learning Explorer about participant feedback during a performance review. In the video, the employee giving feedback was presented with sections where they could give an overall rating and comment on individual goals. Is it possible to limit the participant feedback template so they are only see the actual feedback questions?

We would like employees to see feedback from other colleagues as part of their performance reviews. We currently have anytime feedback enabled, but I would also like to explore using participant feedback.

The business requirement would be for employees to send out feedback requests specifically for their performance reviews, but we would not want the person giving feedback to provide an overall rating or see any information relating to goals.
