Actual Response Time

Hi,

Need help to design a report which will give the actual response time of incidents. I checked couple of standard reports but all of them either give the Create to close time or first response time but not the response time for each transaction. I tried designing a report with incident and transactions table but not able to figure out. I have attached an example ( output should look like that) for better understanding.
