Generating radar chart from custom fields

I have a number of fields available on a incident workspace. They are menus with options 1 to 5. I need to be able to get these values (based on a single incident) plotted onto a radar chart.

The issue is (as I understand it) a radar chart needs two fields: one with the list of the "categories" and another with the value/score and  each category is linked to a score.

On an individual incident a unique field (e.g category) can only hold one value  which is why I had to create a field for each category that holds the value. I have attached a screenshot of the workspace with the fields.
