Can someone help me with the following features/functionality of the All-In-One Evaluation

1. Could it be possible to see this on the box Description, rating, comment, Performance Rating Description and could we also see ‘next’ button so it brings up the next Key Performance Areas (competency) to be evaluated? (see attached picture named: 1_AllinoneEvaluation)
2. Would it be possible to combine the three sections (Outputs, Key Performance Areas, and Overall Performance Summary) into a continuous row or ratings/comments that need to be provided? If so, what functionality? (see attached picture named: 2_AllinoneEvaluation)
